Grief can feel like love with nowhere to go—and you don’t have to carry it alone. This intake helps us understand what you’re moving through and what kind of support would feel most helpful (space to process, tools for grounding, guidance around guilt/regret, or ritual and meaning-making). If you’re not sure where to start, share what you can—we’ll take it one step at a time.

Holistic Healing

For acupuncture visits and integrative care. We’ll ask about your pet’s main concerns, prior treatments, and any helpful records. We also collect a comprehensive Traditional Chinese Veterinary Medicine (TCVM) history so we can tailor an individualized treatment plan.

Telemedicine Request

For virtual consults, follow-ups, case review, and guidance when an in-person visit isn’t needed (or while you’re deciding next steps). We’ll ask a few focused questions so we can make the consult efficient and helpful.
